Print and complete the PDF form. Attach the rebate form to your invoice and mail them to our rebate centre. Please note that claims submitted through mail submission will have longer processing times when compared with the online submission method.

    Fill out the online form and click the "Send Form" button to instantly submit your claim. Please note that one claim form is required for every set of four (4) tires purchased.

  • Where do I find my dot number?
    By filling in the DOT Tire ID Number information, you are registering your tires to enable us to contact you in the unlikely event of a recall. The DOT fields are optional only and do not need to be filled out in order to successfully have your claim processed.
